St. Francis Sports Physical Night – Wednesday, June 5, 2019 5:00-8:00 p.m.

St. Francis will be hosting Sports Physical Night for the 2019-20 school year on June 5, with registration starting at 5:00 p.m.  The cost of the physical is $25 per student. It is payable with credit card or check or can be prepaid online. If you have been grandfathered in as a lifetime Booster member, your physical is free if you present your membership card.

We will have an army of Troubie parents who volunteer their time to complete the yearly, required, sports physical your child must have to participate in Athletics. While this event is not mandatory to attend, it is a great opportunity to get the required task out of the way for the 2019-20 school year. Students will go through a full “well child” pre-participation exam administered by a licensed physician.  Before participating in the exam, students will be asked to fill out a health history questionnaire, and have their vitals measured.

Please be aware, if you are not accompanying your child to the event, that you must complete and sign the permission slip in advance, as well as sign the bottom of the health history on the physical form. Both forms can be found below.

The Troubie store will be open during the event. Chromebook pick up will be held in the Technology Department.

We will also be offering Diocesan-approved fingerprinting services ($45) in the Faculty Lounge. If you plan to volunteer on or off campus, this is the cheapest option to have your fingerprints done for the Diocese.  Please note, fingerprinting only needs to be completed once during your time as a St. Francis parent. If you were fingerprinted for a diocesan elementary school, you do not have to be re-printed.

If you choose to have your physical completed by your doctor, please note that all exams must take place after June 1, 2019, and forms are due to the Athletic Department before participating in a fall sport and/or summer conditioning. Please have your physician complete the St. Francis form found on our website or at the link listed below. If your daughter has a pre-existing, known, medical condition, it would be a good idea to schedule her physical with her personal physician.
