Immunization records must be on file by August 11, 2020.
Before students can be admitted to a private or public California school, California law requires that an immunization record be presented to the staff by the parent or guardian. A complete immunization record must include the Tdap (whooping cough) booster administered on or after your child’s 7th birthday. State law requires us to withhold students from attending until a complete immunization record is on file. You may do one of the following on or prior to August 11, 2020:
- Bring a copy of your daughter’s immunization record to Freshman Registration Night.
- Fax a complete copy of your daughter’s immunization record to 916-452-1591, making sure your daughter’s name is clearly written on the fax.
- Email a scanned copy to email@example.com, making sure your daughter’s name is clearly written on the scan.
- Mail or deliver a complete copy to the attention of the Registrar’s Office at 5900 Elvas Avenue Sacramento, CA 95819.
In accordance with SB 277, Personal Belief Exemptions are no longer acceptable. Medical Exemptions signed by a medical professional will continue to be accepted.
