St. Francis Sports Physical Night — Wednesday, June 1, 2017 6-8 p.m.

St. Francis will be hosting Sports Physical Night for the 2017-18 school year on June 1.  The cost of the physical is $20 per student. If you have been grandfathered in as a lifetime Booster member your physical is free if you present your membership card.

Please be aware, if you are not accompanying your child to the event, you must complete and sign permission slip in advance as well as sign the bottom of the health history on the physical form. Links to both forms can be found below.

The Troubie store will be open during the event, and we will be hosting our ever popular Used Uniform Sales in the CLC. Gates will open promptly at 6 p.m.

Coaches from our sports teams will be present to answer any questions you may have about summer workouts, conditioning and tryouts. We will also be posting this information as we receive it on the St. Francis website under the Athletics heading and tryout tab

We will also be offering Diocesan-approved fingerprinting services ($44), if you plan to drive other students to athletic events or volunteer on campus this is the cheapest opportunity to have your fingerprints done for the Diocese.  Please note, fingerprinting only needs to be completed once during your time as a St. Francis parent. If you were fingerprinted for a diocesan elementary school, you do not have to be re-printed.

If you choose to have your physical completed by your doctor, please note that all exams must take place after June 1 and forms are due to the Athletic Department by Aug 1 if participating in a Fall Sport. Please have your physician compete the St. Francis form found on our website or at the link listed below. If your daughter has a pre-existing known medical condition, it would be a good idea to schedule her physical with her personal physician.
